Measure the instant coffee into a small bowl or measuring cup. Add the hot water and stir vigorously with a small whisk or spoon until the coffee granules are fully dissolved and the mixture is slightly foamy.
Stir in the vanilla extract and the sugar (if using) to the coffee concentrate until dissolved. Taste and adjust sweetness as desired. If you want a cooler drink fast, let the concentrate sit 1–2 minutes to cool slightly.
Fill a tall glass about three-quarters full with ice cubes. Pour the milk over the ice, leaving room at the top for the coffee.
Slowly pour the prepared coffee concentrate over the milk and ice. For a pretty layered look pour gently over the back of a spoon. Stir to combine just before drinking if you prefer an even flavor.
Serve immediately. To make ahead, double the coffee concentrate, chill it in the refrigerator for up to 48 hours, and mix with milk when ready.
